For urgent care, please walk into our Urgent Care center directly – no appointment booking is necessary. If you have immediate questions, please call us at the number given below; there's no need to fill out the registration forms beforehand.

Step 1 : To schedule your appointment with SOM Medical Practice, please begin by completing our secure New Patient Registration form online. This initial step allows us to gather essential information to provide you with the best possible care.
Step 2 : Once you have successfully submitted the registration form, you will be automatically redirected to our online appointment booking page where you can select your desired date, time, and provider. If you are not taken automatically to the appointment booking portal, please use the "Request Appointment" link given for each service.
