Dr. Santanu Som is a highly skilled and experienced board-certified family medicine physician. He has a diverse background, having worked as a hospitalist, in general surgery, in emergency medicine, along with other specialties. He is originally from India and has since become a citizen of the United States. In his personal life, Dr. Som is a proud parent of twin children, a boy and a girl. In his spare time, he enjoys both working and watching movies. Dr. Antonino Carlino is a Buffalo native, Doctor of Chiropractic, who is a Fellow in the International Academy of Chiropractic Neurology and a Diplomate in the International Board of Electrodiagnosis. He is also certified in Intra-Operative Neuromonitoring, where he has assisted in several thousand complex brain and spinal surgical interventions, including spinal fusions and tumor/cancer removal. Dr. Carlino is also certified in Manipulation Under Anesthesia and has been in private practice since 2004.

His background and experience has enabled him to work closely and collaboratively with many of the top neurosurgeons and orthopedic surgeons, throughout the state of New York and Florida. Dr. Carlino has 23 years of experience in practice management, practice building, and patient management. With his extensive knowledge of medical treatment protocols, in conjunction with targeted marketing to relevant and varied medical disciplines, he has personally managed, and assisted in establishing medical practices throughout the Western and Central New York areas. He has been very successful at treating patients with various types of highly complex spinal and extremity conditions, as well as those who have failed with surgical interventions.

Lastly, Dr. Carlino has worked as an Adjunct Assistant Professor at New York Chiropractic College and continues to educate interns in at multidisciplinary clinical setting, providing students with hands-on, clinical experience in patient care, practice management, marketing, diagnosis, as well as medical billing and coding.

Johnna Coleman graduated from Holly High School in 1999 then left for the US Navy as a hospital Corpsman. After returning home in 2010, she attended D’Youville College for her BSN/MSN Family Nurse Practitioner. After graduation, she worked at Southtowns Pediatrics for 3 years, Grand Island Family Practice for 2 years, then started at Niagara Primary Care and Urgent Care in 2021.

She resides in the Southtowns with her family and enjoys working out, singing, and reading anything non-fiction.

Cheryl Patterson Reeves was born and raised in Medina, NY, with deep family roots in Orleans County. She pursued her college education at Eastern Mennonite College in Harrisonburg, VA, before beginning her career as a counselor at a domestic violence shelter in Sterling, Illinois. Cheryl later moved to Iowa City, Iowa, where she worked as a research assistant and married her husband, who was raised there. She graduated from the University of Iowa College of Medicine Physician Assistant Program and went on to practice internal medicine in Davenport, Iowa, where her son Jake was born.

Cheryl later moved to Harper, Iowa, a small town of 120 people, working in internal medicine and family practice clinics. During this time, her daughter was born in Ottumwa, Iowa. In 2016, she returned to Medina, NY, to care for her mother. Since then, Cheryl has worked locally and at the Pine Ridge Indian Health Service hospital, an experience she found deeply rewarding.

Amanda Fajardo is an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) with a Master of Science in Nursing (MSN) from Daemen University. During her training, she gained extensive clinical experience in primary care settings. Her approach emphasizes delivering compassionate care that aligns with each individual's goals and values.

In her free time, she enjoys spending time with her husband, children and their golden retriever. She is proud to be a part of the Som Medical Practice team, and to partner with patients to maintain optimal health and quality of life.

Corinna Donner is a caring and experienced Psychiatric Mental Health Nurse Practitioner with a decade of practice. She helps people aged four and up facing mental health challenges and medical diagnoses, offering personalized care to meet their needs. After graduating from Niagara Wheatfield High School in 2005, Corinna served in the military for 12 years, earning an Honorable Discharge in 2017.

During her military service, she pursued her BSN/MSN in Psychiatric Mental Health Nurse Practitioner studies at D’Youville University. Corinna believes in treating each person with respect and dignity, focusing on their individual needs and preferences. She specializes in Child and Adolescent Psychiatry, striving to reduce the stigma around mental health and ensure everyone has access to quality care.

Outside of work, Corinna is a proud wife to a North Tonawanda Firefighter and a devoted mother of four. Her dedication to her family and her patients reflect her compassionate nature and commitment to making a positive difference in the lives of others.

Jody Joustra Grahn attended Alfred University in Alfred, NY for her Bachelor’s Degree where she received the Award of Top Female Senior. She continued on to get her Master’s degree at Nell Hodgkins Woodruff School of Nursing in Atlanta, GA. Jody has over 20 years of experience as a Nurse Practitioner.

In her spare time, Jody enjoys cooking, spending time with her family, gardening, entertaining and baking. She makes the best pecan pie in all of WNY!

Alisa is a compassionate provider, who obtained her BSN from Daemen in 2013. She went on to practice at a medical facility for 2 years, followed by working as a RN at ECMC’s psychiatric emergency room for 7 years. While working as an RN, she attended D’youville, obtained a master's degree as a Psychiatric Mental Health Nurse Practitioner, followed by board certification. As a nurse practitioner, she worked on an Assertive Community Treatment team, serving the community on a mobile unit, as well as working in a clinic setting, serving clients 13+.

Her clinical experience ranged from ages 4 – 95 years of age. She has specialized in depression, anxiety, mood disorders, and those with psychotic illness, in addition to clients with coexistent substance use disorders.

Sarah Arno is a Registered Nurse with three years of experience in medical-surgical and pediatric care. She is known for her calm, compassionate approach and her ability to connect with patients of all ages. Her background reflects a strong foundation in acute care, balanced with a deep understanding of family-centered support.

Outside of nursing, Sarah is a devoted mom to four kids, along with a dog and a lively flock of chickens. She enjoys practicing yoga, spending time outdoors, and is also a skilled fine line tattoo artist, blending precision and creativity in both her professional and artistic work.

Dr. Puneet Sayal is a pain medicine physician double-board certified in Pain Medicine and Anesthesiology. He completed his anesthesia training at Harvard University and his pain medicine trying at MD Anderson Cancer Center. He brings years of experience treating patients with complex pain and medical issues. He is interested in providing comprehensive pain care using minimally invasive methods to treat your pain. Dr. Giglio completed a neurology residency at State University of New York Upstate, in Syracuse, NY. His internal medicine internship was served at the University of Massachusetts Memorial Medical Center, Worcester, MA. In addition, he completed a neurobehavioral fellowship at Strong Memorial Hospital, Rochester, NY and earned his medical degree from Ross University School of Medicine. Dr. Giglio diagnoses and treats disorders of the nervous system. Some commonly encountered conditions he treats include headaches, radiculopathy, neuropathy, stroke, dementia, Alzheimer's disease, Parkinson's disease, multiple sclerosis, epilepsy, spinal deformities, head traumas, sleep disorders and neuromuscular diseases. Dr. Troy Sasse is a board-certified Family Medicine physician with more than 25 years of experience providing compassionate, comprehensive care to patients of all ages. He is dedicated to building lasting relationships with his patients and believes in treating the whole person through preventive care, chronic disease management, and personalized treatment plans. Throughout his career, Dr. Sasse has served in a variety of clinical and leadership roles, including Medical Director for multiple primary care and urgent care practices. His extensive background includes family medicine, urgent care, emergency medicine, occupational health, and medication-assisted treatment (MAT) for patients with substance use disorders. His diverse experience allows him to provide high-quality care for a wide range of acute and chronic medical conditions. In addition to his clinical practice, Dr. Sasse has held faculty appointments, mentoring family medicine residents, physician assistants, and medical students. He has also led quality improvement initiatives, practice development, physician recruitment, compliance programs, and electronic medical record implementation, helping healthcare organizations improve patient care and operational efficiency. Dr. Sasse earned his Doctor of Osteopathic Medicine degree from the University of Health Sciences in Kansas City, Missouri, and completed his Family Medicine residency through Michigan State University. He maintains current certifications in Advanced Trauma Life Support (ATLS), Advanced Cardiovascular Life Support (ACLS), and Pediatric Advanced Life Support (PALS). He is Board Certified in Family Medicine and is Board Eligible in Addiction Medicine. Known for his approachable and patient-centered style, Dr. Sasse is committed to helping individuals and families achieve their best health through evidence-based medicine, education, and compassionate care.
