Dr. Santanu Som is a highly skilled and experienced board-certified family medicine physician. He has a diverse background, having worked as a hospitalist, in general surgery, in emergency medicine, along with other specialties. He is originally from India and has since become a citizen of the United States. In his personal life, Dr. Som is a proud parent of twin children, a boy and a girl. In his spare time, he enjoys both working and watching movies. Overall, Dr. Som is a dedicated and compassionate healthcare professional who is dedicated to providing the highest level of care to his patients.
Dr. Vikram Arya is a distinguished and highly skilled Internal Medicine Board Certified physician. He brings a wealth of experience and expertise to the field, having completed his Internal Medicine Residency at St. Barnabas Hospital. Dr. Arya performs preventive medicine and diagnostic assessments and treats numerous conditions such as hypertension, diabetes, substance abuse, asthma, and hypothyroidism. Embracing modern healthcare practices, Dr. Arya offers virtual visits, allowing patients to access expert medical care from the comfort of their homes. Languages spoken: English.

Johnna Coleman graduated from Holly High School in 1999 then left for the US Navy as a hospital Corpsman. After returning home in 2010, she attended D’Youville College for her BSN/MSN Family Nurse Practitioner. After graduation, she worked at Southtowns Pediatrics for 3 years, Grand Island Family Practice for 2 years, then started at Niagara Primary Care and Urgent Care in 2021. She resides in the Southtowns with her family and enjoys working out, singing, and reading anything non-fiction.
Jody Joustra Grahn attended Alfred University in Alfred, NY for her Bachelor’s Degree where she received the Award of Top Female Senior. She continued on to get her Master’s degree at Nell Hodgkins Woodruff School of Nursing in Atlanta, GA. Jody has over 20 years of experience as a Nurse Practitioner. In her spare time, Jody enjoys cooking, spending time with her family, gardening, entertaining and baking. She makes the best pecan pie in all of WNY!
Jean Hayward has over 7 years of experience as a Family Nurse Practitioner with over 17 years of healthcare experience in family practice, geriatrics, rehab and wound care. She received her Nursing Degree from Jamestown CC, her bachelor’s degree from Jacksonville University, and her master’s degree for FNP from Saint Joseph's College. She worked fulltime as a RN charge nurse while successfully completing her degrees and become a board-certified medical provider. She earned her MBA and worked as a financial analyst/stock broker. Today, she feels fulfilment after making the change in her career to a medical provider. When she’s not working, Jean enjoys traveling, singing, spending time with her family. She has served the church as a Eucharistic minister, early childhood religious education teacher, and an intergenerational religious education instructor. She earned her Doctor of Divinity degree and she received the St. Joseph’s service award from the Bishop of Buffalo. She is currently working on her DNP, “.. the more I know the better it will be for my patients." She believes in the words of Mark Twain, “Kindness is a language which the deaf can hear and the blind can see.”
Tiffany Fabian started off her career with a degree with a B.S. in business and then a Master's in Social Work from Buffalo State College. She quickly learned that she wanted a more hands on approach to patient care; therefore, while working full-time, Tiffany went to school to get her Registered Nursing Degree from Erie County Community College, and then went on to obtain her B.S. in Nursing from Daemen College. Finally, her educational journey ended at the University of Buffalo where she obtained her doctorate in Adult Nurse Practitioner. She has been an RN since 2009 and a Nurse Practitioner since 2021. Tiffany especially enjoys listening to her patients, taking her time with them, and providing exceptional patient care.

Michael Wiewiorski is a Physician Assistant (PA) with over 16 years of expertise in acute and chronic care for patients of all ages. He graduated from D’Youville College Physician Assistant Program in Buffalo, New York, and is certified by the National Commission of Certification of Physician Assistants. He is a certified healthcare professional who is driven by challenges, undeterred by obstacles, and committed to furthering standards set before him. He served on active duty as a Hospital Corpsman and also as a Flight Surgeon during Operation Iraqi Freedom in Kuwait and Iraq, providing vital medical care to soldiers and civilians, and retired from the US Army. Michael is committed to serving others, and driven by a passion to deliver services that go beyond expectations.
Cheryl Patterson Reeves was born and raised in Medina, NY, with deep family roots in Orleans County. She pursued her college education at Eastern Mennonite College in Harrisonburg, VA, before beginning her career as a counselor at a domestic violence shelter in Sterling, Illinois. Cheryl later moved to Iowa City, Iowa, where she worked as a research assistant and married her husband, who was raised there. She graduated from the University of Iowa College of Medicine Physician Assistant Program and went on to practice internal medicine in Davenport, Iowa, where her son Jake was born. Cheryl later moved to Harper, Iowa, a small town of 120 people, working in internal medicine and family practice clinics. During this time, her daughter was born in Ottumwa, Iowa. In 2016, she returned to Medina, NY, to care for her mother. Since then, Cheryl has worked locally and at the Pine Ridge Indian Health Service hospital, an experience she found deeply rewarding.
Prashant Sharma is a skilled physician assistant who brings dedication and expertise to patient care across a diverse age range. With a focus on addressing both acute and chronic health conditions, Prashant is committed to fostering a compassionate and supportive environment for each patient. His approach emphasizes empathy, ensuring that all patients feel heard and respected as they navigate their healthcare journeys. Prashant's dedication to improving patient outcomes makes him an invaluable part of the healthcare team.
Corinna Donner is a caring and experienced Psychiatric Mental Health Nurse Practitioner with a decade of practice. She helps people aged four and up facing mental health challenges and medical diagnoses, offering personalized care to meet their needs. After graduating from Niagara Wheatfield High School in 2005, Corinna served in the military for 12 years, earning an Honorable Discharge in 2017. During her military service, she pursued her BSN/MSN in Psychiatric Mental Health Nurse Practitioner studies at D’Youville University. Corinna believes in treating each person with respect and dignity, focusing on their individual needs and preferences. She specializes in Child and Adolescent Psychiatry, striving to reduce the stigma around mental health and ensure everyone has access to quality care. Outside of work, Corinna is a proud wife to a North Tonawanda Firefighter and a devoted mother of four. Her dedication to her family and her patients reflects her compassionate nature and commitment to making a positive difference in the lives of others.
Jenna Carroll is a dedicated Psychiatric Mental Health Nurse Practitioner with a wealth of experience across diverse healthcare settings. Her journey began at the University of Alabama, where she earned a Bachelors of Psychology with a specialization in Drug and Alcohol Use. Building on this foundation, Jenna pursued her Bachelors in Nursing through Niagara University's Accelerated Nursing Program, achieving her RN, BSN. Her career as a nurse specialized within the mental health sector working within various departments, from adolescents to adults and geriatrics, spending the majority of her time on the psychiatric intensive care unit. Jenna continued her education at D'Youville University, where she earned a Master's of Science in Psychiatric Mental Health Nurse Practitioner. Since then, she has made significant contributions in both inpatient hospital settings and outpatient clinics, providing compassionate and evidence-based care to her patients. Jenna is known for her expertise, empathy, and dedication to improving the mental health and well-being of her patients, utilizing her extensive experience to deliver comprehensive psychiatric care.
James Sheehan (Jim) is a highly experienced Mental Health Counselor with nearly 40 years in the field, specializing in addiction recovery. Holding master's degrees in Counseling and Bioethics, along with a CASAC-2 certification and a Doctor of Divinity degree, Jim brings a holistic and spiritual approach to well-being. In addition to addiction recovery, Jim works with individuals dealing with codependency, domestic violence, grief, self-esteem, intimacy/relationship issues, stress, spirituality, trauma, and LGBTQ+ concerns. Theresa Quaglia, an LPN for over 35 years, has been a valuable asset to Som Medical Practice since 2022. In her role as a Chronic Care Manager, Theresa excels in coordinating and managing the care of patients with chronic health conditions. She is known for her ability to build strong relationships with patients, offering support and guidance to help them navigate the complexities of managing chronic illness. Theresa's dedication to staying current with the latest developments in healthcare ensures that she provides evidence-based and up-to-date care to all her patients. With a career spanning decades, she brings a wealth of experience, expertise, and a genuine passion for improving the lives of patients. Outside of her professional life, Theresa is a loving wife to a Navy veteran, a devoted mother of two, and a proud grandmother of two.
