What insurance plans do you accept?

We accept more major insurance plans. Please check the page of your preferred location to view accepted insurances.

How do I transfer my medical records to your office?

To transfer medical records, fill out our Records Request Form, available online or at any of our offices.

How do I transfer my medical records from your office to a new office?

Your new doctor's office must fax us a records request form.

What is the status of my medical record request?

What should I bring to my appointment?

Please bring a valid photo ID, your insurance card, a list of medications you’re taking, and any relevant medical records.

Do all locations offer the same services?

No, not all services are available at every location. Please check the page of your preferred location for specific details.

How can I book an appointment?

Appointments can be scheduled online here or by calling your preferred location.

I got my lab work done. How long does it take for test results?

Most routine lab work results are available within 24 to 72 hours. Specialized tests may take longer.

Do you perform DOT Physicals?

No we do not.

What is the charge for a self pay visit?
  • For urgent care, self pay visits typically range from $150 to $200.
  • For primary care, self pay visits typically range from $85 to $250, depending on the type.
  • For psychiatry, self pay visits typically range from $250 to $350.
How do I pay by bill?

Bills can be paid online, by phone, or in person.

What is your policy on missed appointments or late cancellations?

We request 24-hour notice for cancellations. Missed appointments or late cancellations will be charged a $50 fee.

2. Pediatric Care

What pediatric services do you offer?

We provide comprehensive pediatric care, including routine check-ups, vaccinations, developmental assessments, and treatment for acute and chronic conditions.

What should I bring to my child's first appointment?

Please bring your child's insurance information, medical records, vaccination history, and a photo ID.

My child is a current patient. How can I schedule an appointment for my child?

Can I get same-day sick visits for my child?

Same-day sick visits are available for established pediatric patients at Healthy Beginnings Pediatrics in Hamburg, NY. You can schedule for a same-day sick visit by calling 716-202-0401.

Can I schedule an appointment for my unborn baby?

Please wait until your baby is born before scheduling an appointment.

Does my child have to be vaccinated to come to your facility?

Whether you decide to vaccinate or not, your child is welcome at our practice.

My child is sick but is not yet a patient at your practice. Can I schedule my child for a sick visit?

Unfortunately, your child must first be established as a patient in order to schedule them for a sick visit. Please bring your child to an urgent care or the ER. Once your child is better, you can schedule a new patient appointment for them.

Does my child have to be vaccinated to come to your facility?

Whether you decide to vaccinate or not, your child is welcome at our practice.

What types of services do you offer?

We provide urgent care for non-life threatening conditions, including illnesses, injuries, and minor emergencies such as sprains, fractures, cold and flu symptoms, and more.

How can I schedule an appointment with a primary care provider?

Appointments can be scheduled online here or by calling your preferred location.

What services are offered at Niagara Primary Care?

We offer primary care services, Chronic Care Management (CCM), and psychiatry (both virtual and in-office). We also partner with Emerald Way to offer Medication Assisted Treatment (MAT) with buprenorphine.

What services are offered at Southern Tier Primary Care?

We offer primary care services, Chronic Care Management (CCM), and virtual psychiatry. We also partner with Emerald Way to offer Medication Assisted Treatment (MAT) with buprenorphine.

What services are offered at Lockport Primary Care?

We offer primary care services, Chronic Care Management (CCM) and psychiatry (both virtual and in-office).

5. Mental Health Care

Can I see a psych provider at Niagara Primary Care?

6. Addiction Treatment

What is Medication-Assisted Treatment (MAT)?

MAT combines medications like buprenorphine with counseling and behavior therapies to treat opioid addiction. Our team provides individualized care to support recovery.

What is the cost for Medication-Assisted Treatment (MAT)?

The first visit costs $200. We collect a non-refundable $50 fee at the time of booking, and the remaining $150 at your first appointment. After that, the cost is $165 at every 4 week follow-up. Call 716-257-1254 to schedule your initial consultation.
