How to Schedule Your Appointment
Step 1 : To schedule your appointment with SOM Medical Practice, please begin by completing our secure New Patient Registration form online. This initial step allows us to gather essential information to provide you with the best possible care.
Step 2 : Once you have successfully submitted the registration form, you will be automatically redirected to our online appointment booking page where you can select your desired date, time, and provider.

Yes, we are accepting new patients.
We offer primary care services, Chronic Care Management (CCM), and virtual psychiatry. We also partner with Emerald Way to offer Medication Assisted Treatment (MAT) with buprenorphine.
MAT combines medications like buprenorphine with counseling and behavior therapies to treat opioid addiction. Our team provides individualized care to support recovery.
The first visit costs $200. We collect a non-refundable $50 fee at the time of booking, and the remaining $150 at your first appointment. After that, the cost is $165 at every 4 week follow-up. Call 716-257-1254 to schedule your initial consultation.
To transfer medical records, fill out our Records Request Form, available online or at any of our offices.
Please bring a valid photo ID, your insurance card, a list of current medications, and any relevant medical records. If your insurance requires a copay, it will be due at the time of service. Arriving 15 minutes early is recommended.
Most routine lab work results are available within 24 to 72 hours. Specialized tests may take longer.

For primary care, self pay visits typically range from $85 to $250, depending on the type.
For psychiatry, self pay visits typically range from $250 to $350.
We request 24-hour notice for cancellations. Missed appointments or late cancellations will be charged a $50 fee.
